Kindergarten Sign Up 2014-2015
If you have a child who will be 5 years old by September 15, 2014, they are eligible for kindergarten here at Samuelson. If you already have a child in Des Moines Public Schools you will need to sign up at Samuelson from beginning Wednesday, March 26th, 2014. Families who are new to the district and have no other children in Des Moines Public Schools are required to sign up their student(s) at the Welcome Center (1301 2nd Ave). Please call the Welcome Center at 242-7371 to set up an appointment.
You will need the following items for Kindergarten Sign Up:
*Proof of birth (birth certificate)
*Proof of immunizations (a current immunization record)
*Photo ID and 2 proofs of address (water bill, power bill, lease agreement, etc.)
Your child will not be able to start kindergarten without these items.
ELL Kindergarten Sign Up takes place on March 3rd and 10th from 4:00-8:00 p.m. in the North High School cafeteria (501 Holcomb Ave). Interpreters will be present to translate and assist with forms. If you have any questions about ELL registration, please call Erika Mendez at 242-8102.
